MQAP Admission Policy

A. Admission Policies for New Students and Transferees

  1. Admission Policy
    As a policy, the Admissions Committee of MQAP admits new students and transferees on the following basis:
    1. Preschool
      Assessment in the form of an oral interview.
    2. Grade School
      Evaluation of Entrance test results, character and academic records.
    3. High School
      Evaluation of Entrance test results, personal interview, character and academic records.
  2. Age Requirements are as follows
    1. Preschool
      1. Nursery Class 3 to 4 years old as of June
      2. Kinder Class 4 to 5 years old as of June
      3. Prep Class 5 to 6 years old as of June
         
    2. Grade School
      Grade 1 6 to 7 years old as of June and/or must have completed the last level in the preschool
         
    3. Admission Requriements
      1. Preschool and Grade One Applicants
        1. Accomplished Registration Form of MQAP
        2. Two (2) pieces 1x1 picture
        3. Photocopy of birth certificate
        4. Photocopy of baptismal certificate
        5. Photocopy of marriage certificate of parents
        6. Original copy of report card from last school attended
        7. Preschool certificate stating that the child is presently enrolled in a Preschool Program ( if available)
        8. Medical Certificate stating that the child is physically fit to study
        9. Photocopy of medical and dental records if available
        10. One (1) long folder (white) with child’s name and level applied for
        11. Testing fee
      2. Grades 2-6 and High school applicants
        1. Accomplished Registration Form of MQAP
        2. Two (2) pieces 1x1 picture
        3. Photocopy of birth certificate
        4. Photocopy of baptismal certificate
        5. Photocopy of marriage certificate of parents
        6. Original copy of report card from last school attended
        7. Medical certificate stating that the child is physically fit to study
        8. Photocopies of Medical and Dental Records if available
        9. Certificate of Good Moral Character
        10. One (1) long folder (white) with child’s name and level applied for
        11. Testing fee
    4. Application Period
      Application for admission in all levels is scheduled from October to May.

B. Re-Admission Policies For Old Students

  1. Year-End Deliberation
    1. Promoted in
      The student had completed and passed all academic and non-academic requirements and is promoted to the next level.
    2. Promoted out
      The student will be promoted to the next level but will be asked to transfer to another school if any of the following exists:
      1. For regular students
        1. Has failing grade in any of the academic subjects.
          Note: Failing mark shall mean a grade of below 75%.
        2. Committed a major offense that requires dismissal as sanction.
      2. For probationary students
        1. Has failing grade in any of the academic subjects.
        2. Committed a major offense that requires dismissal as sanction.
        3. Has a conduct grade below 85%.
        4. Has a grade below 80% in any of the academic subjects.
    3. Failed out
      The student is asked to repeat the same level in another school because of failing marks indicative of his inability to handle the academic requirements of MQAP.
      1. Grade school - failure in more than three (3) academic subjects.
      2. High school - failure in two (2) or three (3 academic subjects or subjects equivalent to three (3) units.
  2. Re-admission for Preschool, Grade School and High School
    Depends on the child’s academic performance and conduct during his previous grade/year level. The conditions for re-admission are as follows:
    1. For regular students
      1. A regular student will be re-admitted and promoted to the next level if he passes all the following requirements:
        1. has no failing grade in any subject.
        2. has obtained a general average of 80% or above.
        3. has obtained a conduct grade of 85% or above.
      2. A regular student will be re-admitted and promoted to the next level under probationary status if he falls to all of the following requirements:
        1. has no failing grade in any subject.
        2. has obtained a general average of 75% to 79%.
        3. has obtained a conduct grade of 80% to 84%.
  3. Inclusion Program

    Mainstreaming Program
    This program caters to children with special needs. The child will be admitted under probation depending on his quarterly performance in school.

    The probationary admission of the child will also be under the following conditions.
    1. The child shall maintain his own shadow teacher for the duration of the school year.
      1. Any arrangement entered into with the shadow teacher will exclusively be between the parents and the shadow teacher.
      2. The shadow teacher will likewise be asked to go through an orientation with the school at the beginning of the school year.
      3. The shadow teacher is not a part of the school faculty, and shall only be present in the school premises while his ward is attending classes.
      4. The role of the shadow teacher is strictly limited to assisting the “special child” in finishing his schoolwork. The shadow teacher is not allowed to teach the answers to his ward nor answer for him. During exams, the shadow teacher will be asked to stay somewhere which is not near the child and will only be called if the need arises.
      5. The child will be allowed to attend regular classes only when he has a shadow teacher.
    2. The child has to undergo regular therapy sessions with a therapist of his own choice.
      1. The therapist shall submit the child’s regular evaluation report to the guidance counselor on a quarterly basis.

      Any deviation from the above conditions will be reflected in the child’s quarterly assessment. A recommendation for a more beneficial/suitable arrangement for the child will be given to the child’s parents should the need arise.
